George Kissen: on integrating care in Trafford

Trafford is currently developing a new approach to care: nine general practices are working with community, acute and social care organisations to redesign selected care pathways, share data, identify complex patients and prevent unplanned hospital admissions. The process is being overseen by an emerging integrated care organisation.
