This slidepack, setting out evidence and experience about integrated care, has been drawn from work by The King’s Fund, Nuffield Trust and others, and covers definitions, evidence, case studies, enablers and facilitators, and overall lessons for the development of more integrated services.

Nuffield Trust Head of Policy Dr Judith Smith and Dr Nick Goodwin, Senior Fellow in Health Policy at The King’s Fund, begin by reflecting on what we mean by integrated care and the problems it seeks to address, before looking at several examples of integration, including integrated care for frail older people in Torbay Care Trust, and the implementation of a pan-London stroke care pathway.

They go on to consider why integrated care is such a challenge, including the key organisational and management barriers; what can be done to support the development of more joined-up and efficient services; and what these experiences tell us about adopting and mainstreaming integrated care ‘at scale’.

This slideshow forms part of the Nuffield Trust and King’s Fund research project: Developing a national strategy for the promotion of integrated care.  The project aims to support the Department of Health in the development of its strategy on integrated care. This slidepack will act as a ‘primer’ for the work, and will be submitted to the Department of Health along with the final report: Integrated care for patients and populations: Improving outcomes by working together.
