The Nuffield Trust has been working with colleagues in Trafford – a borough of 215,000 people in Greater Manchester – to carry out an in-depth study tracking the development of a new integrated care system.
This presentation of key slides from the first phase of the research provides an overview of Trafford’s experiences over the last three years as they have worked to develop integrated care.The drivers for change – such as the financial issues faced by the borough – are explored, as are the key principles being used to guide integration, for example keeping the patient voice at the heart of plans for integrated care.
The slides illustrate Trafford’s integrated governance arrangements and emerging plans for an integrated care organisation. The mechanisms for engaging patients and the public in Trafford are outlined, alongside plans for closer working across health and social care.
The slideshow concludes with an overview of the lessons to be learned from Trafford’s experience, including ensuring a clear and agreed vision, being realistic about the pace of change, and actively engaging clinicians across primary and secondary care.
