David entered medical school in 1947, one year before the National Health Service was introduced, qualified in 1952 and entered general practice in 1957.

At that time diagnosis was defined in the Concise Oxford Dictionary as 'the identification of disease by means of patients' symptoms, etc'; a similar definition was to be found in the most reputable dictionaries at the time and has not changed since.
