Initiative and inertia: Case studies in the NHS

These are not all from policy initiatives that have been centrally inspired, either politically or professionally, for many have had more humble origins, often beginning with a single individual's ideas which because of their intrinsic worth have attracted widespread support. It is clearly important to understand how these ideas mature and influence policy and practice.
