A medical questionnaire is concerned with symptoms, perceived changes indicating disease and communication. Whilst communication is difficult to define in a universally acceptable way, the word communicate is more easy.

This report explores how questionnaires affect the understanding of doctor and patient, whether using written or verbal systems for communicating.
