The enquiry investigated all deaths within 30 days of an operative procedure in order to clarify the current 'state of the art' of surgery and anaesthesia throughout three Regional Health Authorities in England in 1986.
Recommendations on how to improve quality of health care and conclusions are provided based upon the findings from the investigation.
