Professor Sir Albert Aynsley-Green was the twelfth Queen Elizabeth the Queen Mother Fellow for the Nuffield Trust. The monograph has been developed from Professor Aynsley-Green’s Fellowship lecture, presented in 2006, to reflect current policy and practice.
